To request a transcript from Creekside High School, you may do so in person or by mail.
In-Person Requests
Visit the Creekside Education Center Administration Office and see our Records Clerk, Jazmeen Khorram.
- A valid photo ID is required
- Cost: $4.00 (cash) or $4.61 (card, in person only)
Mail Requests
Send the following items to:

Please include:
- A written request for your transcript
- A $4.00 money order
- A copy of your valid photo ID
- The complete address where the transcript should be sent
